  • Audit:

    • I initiated and participated in few audit projects as follows
      • I participated in audit on “Outcome of joint replacement in obese patients” at Dumfries and Galloway Royal Infirmary with Mr Sameh Hatab consultant orthopaedic surgeon. The project is in progress.
      • I established the Egyptian Arthroplasty Register 2007, which is now an associate member of the International Society of arthroplasty Registers (ISAR). http://www.isarhome.org/directory
      • This is the start of a bigger aim to have a national register and to audit the outcome and the process of arthroplasty services in Egypt, which currently not only require a lot of funding that is not affordable but also have unsatisfactory outcome. Few audit projects were based on the registry data. Two projects had a completed audit loop.
        1. Need for blood transfusion in unilateral and bilateral TKR: Done with anaesthetists and our quality manager prospectively reviewing 100 cases of TKR. We identified that only 5% of unilateral and 16 & of bilateral required blood transfusion. We stopped routine booking and transfusion of blood. This reduced the cost and possibly the risk of hepatitis B and C, which is endemic in Egypt. Some changes were made to preoperative assessment, hip and knee class and the fast track arthroplasty (details are written in a full paper, ready for publication).
        2. Outcome of hip and knee arthroplasty at October 6 University looking at 500 cases with view to the survival and performance of implants. We identified hip and knee implants with poorer outcome. We did FMEA and root cause analysis for failure. Some implants were discontinued. Data presented in the Annual meeting of ISAR in Boston, MA, USA, 31May - 2 June, 2014.
        3. Audit on the risks and benefits of bilateral knee Arthroplasty
        4. Audit of infected joint arthroplasty including regional hospitals In addition to a morbidity and mortality meetings, I participated in audit research in the form of a clinical trial “The benefits and safety for knee intra articular injections of Hyaluronate” http://www.clinicaltrials.gov/ct2/show/NCT01998308?termhafez@msn.comm=mhafez&rank=1
        5. Audit Research on the outcome of cemented stainless steel Furlong THR" This was conducted at Dewsbury District Hospital. It was resulted in reassurance about the quality of care that required no changes. Presented in EFORT 2003,abstract published in JBJS-Br proceedings 2004
        6. "Audit of radiation risk to find out risk of radiation to the hands of orthopaedic surgeons at different level of experiences; consultants and trainees" 1999. The risk was found to be higher than what was expected and what was reported in the literature. Results were applied with the raise of awareness and increase level of protection. It was published in archive of orthopaedics Journal 2005.
        7. "Audit of fracture neck of femur" This was conducted at Homerton Hospital, London, UK and presented at The Departmental Annual Meeting on 26 Jan. 1996. It resulted in a fast track admission from A & E.
